1. Permission/Medical/Media Release (Yellow form)
This form needs to be printed on white paper, filled out, signed and
returned to the Youth office. It is a signed, written consent form giving
your personal information, medical information and media release to be
in youth activity photos to be put on the church website or used in any
other media outlets used by the church. Call us if you have

2. Youth Volunteer Liability Release Form
This form needs to be printed on white paper, filled out, signed and
returned to the Youth office. It is a signed, written parental consent
form giving
permission for a youth to assist Clear Lake United Methodist
Church (CLUMC) in performing clean up, construction and /or demolition
in order to help provide disaster relief for any affected areas Call us if
